For entry into the European phase, applicants are strongly recommended to use special form EPO/EPO/OEB Form 1200 (Annex X).

The form is obtainable from the EPO or may be downloaded from the EPO website (http://www.european-patent-office.org/epo/formul/epc_e.htm). Applicants are advised to read the explanatory notes carefully before completing the form.

The form must be filed with the EPO direct (not the IB) either by post or by fax. Applicants must indicate the European application number, which the EPO communicates to them approximately two months before expiry of the international phase. Applicants who have not yet been notified of this number when performing the required steps may instead cite the PCT application number or the PCT publication number.
