Is there any fee associated to the request?

No fee is payable for requests for entry on the list of professional representatives. Once you have been entered on the list, however, membership of the Institute of Professional Representatives before the EPO (epi) is mandatory (Article 134a(2) EPC) and an annual subscription is payable.  https://patentepi.org/en/the-institute/annual-subscription.html
