INFORMATION FROM THE EPO
Notice dated 31 May 2000 concerning new versions of various forms
1. Most EPO application forms have been revised of late. The latest versions are reproduced below in the following order1:
- Designation of inventor (EPA/EPO/OEB Form 1002 02.00),
- Authorisation (EPA/EPO/OEB Form 1003 02.00),
- Receipt for documents (relating to international applications) (EPA/EPO/OEB Form 1031 01.99),
- Acknowledgement of receipt for subsequently filed items (EPA/EPO/OEB Form 1037 03.99),
- Letter accompanying subsequently filed items (EPA/EPO/OEB Form 1038 04.00).
2. The current versions of the forms Request for grant (EPA/EPO/OEB Form 1001 07.99) and Entry into the European phase (EPA/EPO/OEB Form 1200 03.00) and relevant notes were published at the time of last revision2. In addition the Notes relating to the version of Form 1200 04.993 that may be used for a transitional period were revised and reprinted under reference 03.00; they are not reproduced here.
3. The use of revised forms is recommended.
4. The forms are obtainable free of charge from the EPO (preferably in Vienna, but also Munich, The Hague and Berlin) and the central industrial property offices of the contracting states.
5. The forms can also be downloaded from the EPO's website (www.european-patent-office.org)4 in pdf and Word 97 formats.
